What Happens During the Exam?
Your appointment will consist of several key components:

After your examination is complete, our Civil Surgeon will fill out Form I-693, the official Report of Immigration Medical Examination and Vaccination Record.
How to Prepare for Your Immigration Physical
Proper preparation can help your appointment go smoothly and prevent delays in your application process. Before you visit us, please gather the following essential items:
- Form I-693: Please download and print the most recent version of Form I-693 from the USCIS website. You should fill out Part 1 of the form but do not sign it until the Civil Surgeon instructs you to do so.
- Government-Issued Photo ID: Bring a valid, unexpired passport or driver’s license. For applicants under 14, a birth certificate (with an English translation if necessary) or an affidavit is acceptable.
- Vaccination Records: Provide a complete record of all your past immunizations. If your records are not in English, you must bring a certified English translation.
- Payment for the Exam: Our immigration physicals start at $275. Please check with our office about acceptable forms of payment.
- Health Insurance Card: While many insurance plans do not cover immigration physicals, it is good to have your card with you.
After the Examination
Once the exam is finished and all results are in, the Civil Surgeon will complete Form I-693, sign it, and seal it in an envelope. You must not open this envelope. It must be submitted to USCIS sealed. We will provide you with a copy of the completed form for your personal records before sealing the official document.
